Form for Research Project Revision as per the Resolution of the Committee
Document Submission: Submit the response to the Committee’s comments via the DOMS system and two (2) original hard copies to the Office.

Form for Post-Approval Research Projects
Document Submission: No hard copy is required. Submit all required documents via the DOMS system for Progress Reports, Continuing Review, Study Closure, and Research File Requests.
Document Submission: Submit the amendment memorandum via the DOMS system and two (2) original hard copies to the Office.
